- Add detectarIntencionKeyword() with regex patterns in Colombian Spanish
  that runs before Gemini (instant, no API cost)
- Gemini is now only called for ambiguous text
- Improve Gemini prompt with real examples and informal Spanish
- Replace generic "no entendí" with actionable buttons
